Job Description

Qualified Professional providing community-based mental health care and case management for SEIC clients in North Carolina. Coordinating care, documenting plans, and promoting safety and recovery.

Responsibilities:

  • Provide community-based care and case management for the Community Support Team (CST) in Robeson County
  • Ensure clients receive high-quality care in their home or community location
  • Coordinate care and advocate for clients
  • Update PCPs and enter documentation into the system
  • Participate in developing PCPs
  • Ensure quality care and safety of individuals served
  • Serve individuals and families through facilities, community settings, or secure virtual care
  • Collaborate with an interdisciplinary team to promote holistic wellness and recovery

Requirements:

  • Must meet the requirements of a NC Qualified Mental Health Professional
  • Minimum of a bachelor’s degree
  • Minimum of 2 years of experience with the population served
  • Current, valid NC driver’s license
  • Insurance with a clean driving record
  • Successful completion of an extensive background check
  • Successful motor vehicle records check
  • Successful drug screening
  • Required education verification
  • Licensure or certification credentialing with SEIC
