Job Description
Qualified Professional providing community-based mental health care and case management for SEIC clients in North Carolina. Coordinating care, documenting plans, and promoting safety and recovery.
Responsibilities:
- Provide community-based care and case management for the Community Support Team (CST) in Robeson County
- Ensure clients receive high-quality care in their home or community location
- Coordinate care and advocate for clients
- Update PCPs and enter documentation into the system
- Participate in developing PCPs
- Ensure quality care and safety of individuals served
- Serve individuals and families through facilities, community settings, or secure virtual care
- Collaborate with an interdisciplinary team to promote holistic wellness and recovery
Requirements:
- Must meet the requirements of a NC Qualified Mental Health Professional
- Minimum of a bachelor’s degree
- Minimum of 2 years of experience with the population served
- Current, valid NC driver’s license
- Insurance with a clean driving record
- Successful completion of an extensive background check
- Successful motor vehicle records check
- Successful drug screening
- Required education verification
- Licensure or certification credentialing with SEIC
