Job Description
Regional Clinical Manager guiding clinical operations and compliance for CenterWell Home Health teams in New York. Supporting clinicians delivering personalized in-home care.
Responsibilities:
- Support clinical operations for multiple locations, including patient care delivery, staff management, documentation quality, and regulatory compliance
- Review referrals, determine admission appropriateness, and ensure Plans of Care meet patient and agency needs
- Guide, support, and educate clinicians on goal-setting, care planning, and clinical decision-making
- Remain available during operating hours for clinical support
- Ensure clinical documentation, audits, and billing meet Medicare, payer, and company standards
- Monitor case management quality and outcomes
- Participate in hiring, training, performance evaluation, coaching, and corrective action for clinical staff
- Conduct ongoing staff education based on documentation review, utilization review findings, and performance improvement data
- Coordinate communication among physicians, team members, and caregivers for care coordination, discharge planning, and outcome achievement
- Participate in quality improvement, data tracking, budgeting, marketing initiatives, and community relationship development
- Perform additional tasks supporting clinical operations and organizational goals
Requirements:
- Graduate of an accredited School of Nursing
- BSN required (NY State requirement)
- Current New York State license as a Registered Nurse; candidates from adjacent states may be considered
- Current CPR certification/proof
- Valid driver's license, auto insurance, and reliable transportation
- Two years as a Registered Nurse with at least one year of management experience in a home health environment
- OASIS experience
- Certified Home Health Agency experience required (New York State requirement), or four years' CHHA experience and six credit hours toward BSN
- Homecare Homebase (HCHB) experience
- Quality Assurance and Performance Improvement experience
- CMS PDGM billing knowledge and/or experience
- Must complete TB screening if selected
- Internet service with at least 25 Mbps download and 10 Mbps upload for home work
- Dedicated work space without ongoing interruptions to protect member PHI/HIPAA information
