Job Description

Care Manager coordinating whole-person behavioral health services for Trillium Health Resources members. Developing care plans and connecting North Carolina members with providers and community resources.

Responsibilities:

  • Provide comprehensive, whole-person care management to members within a managed care environment.
  • Serve as the primary point of contact for assigned members, providing ongoing care management and advocacy.
  • Conduct comprehensive assessments of member needs using person-centered planning methods and organizational tools.
  • Develop, implement, and monitor individualized care plans with members, families, and multidisciplinary support teams.
  • Coordinate services across providers, agencies, and community resources to address physical, behavioral, and social health needs.
  • Ensure assessments and care plans are updated to reflect changes in member needs and circumstances.
  • Ensure alignment with regulatory and organizational requirements.
  • Travel within the catchment as required.

Requirements:

  • Must meet QP status.
  • Fully licensed by the NC governing board regulating Human Services professions; OR fully licensed RN licensed to practice in North Carolina by the NC Board of Nursing with four years of full-time experience with the MH/DD/SA population served; OR master's degree in a human service field with one year of full-time MH/DD/SA experience or qualifying supervised substance-abuse counseling experience; OR bachelor's degree in a human service field with two years of full-time MH/DD/SA experience or qualifying supervised substance-abuse counseling experience; OR bachelor's degree in another field with four years of full-time MH/DD/SA experience or qualifying supervised substance-abuse counseling experience.
  • If serving members with LTSS needs, must additionally have at least two years of prior LTSS and/or HCBS coordination, care delivery monitoring, and care management experience; this may be concurrent with QP experience.
  • License and/or certification may be accepted as noted above.
  • Must have a valid driver's license.
  • Must reside in North Carolina and within Trillium's South Central Region or within 75 miles, including Carteret, Craven, Duplin, Jones, Lenoir, Onslow, Pender, Sampson, and Wayne counties.
  • Must be able to travel within catchment as required.
  • All candidates must pass a drug test as a condition of employment.
