Job Description
Case Manager coordinating person-centered care plans and support services for Humana's Wisconsin Family Care members. Serving frail elders and adults with disabilities through remote case management and field visits.
Responsibilities:
- Assess members' health and safety needs and develop comprehensive Member Care Plans
- Coordinate with a Field Care Nurse to provide services according to each Member Care Plan
- Conduct face-to-face social assessments upon enrollment and at least every six months, typically at members' residences
- Conduct quarterly in-person visits and maintain monthly phone contact
- Arrange support services related to social integration, community resources, employment, housing, and other non-medical needs
- Ensure cost-effective service delivery
- Evaluate risk factors and provide education to members
- Maintain accurate case notes, service authorizations, and Member Care Plan updates
- Serve members in the Wisconsin Family Care program across Calumet, Outagamie, and Winnebago counties
- Travel up to 40%
Requirements:
- Bachelor's degree in human services or a related field and at least 1 year of experience serving frail elders or adults with intellectual, developmental, or physical disabilities
- Alternatively, Bachelor's degree in another field and at least 3 years of such experience
- Valid state driver's license
- Proof of personal vehicle liability insurance with at least 100/300/100 limits
- Reside within 45 minutes or less of the assigned coverage area (preferred)
- Case Management experience (preferred)
- Experience with electronic case note documentation (preferred)
- Knowledge of community health and social service agencies and additional community resources (preferred)
- Ability to work Monday–Friday, 8:00 am–4:30 pm CST
- Minimum home internet speed of 25 Mbps download and 10 Mbps upload for home or hybrid home/office employees
- Ability to work from a dedicated space without ongoing interruptions to protect member PHI/HIPAA information
- Must complete TB screening if selected
