Job Description
System of Care Family Partner helping North Carolina families navigate child mental health, substance use, and social services. Coordinating person-centered care and community-resource access for Trillium Health Resources.
Responsibilities:
- Educate parents/caregivers on navigating child-serving physical, behavioral, and social service systems
- Inform parents/caregivers about informal and community resources
- Facilitate parents’/caregivers’ access to available resources
- Work alongside the System of Care Coordinator in assigned counties
- Support a system of care for young people and families receiving child mental health and substance use services
- Ensure members receive whole person care addressing physical health, behavioral health, and social determinants of health
- Participate in System of Care planning, implementation, coordination, and training
- Provide education and support regarding members’ and families’ rights and responsibilities
- Serve on multidisciplinary care teams using person-centered planning methods
- Support discovery activities, information gathering, and assessment
- Assist members or legally responsible persons in choosing service providers objectively
- Support members/families in directing the planning process and plan development
Requirements:
- High School Diploma or GED
- Minimum of four (4) years of experience as a primary caregiver for a child or youth receiving Medicaid or State funded BH services
- Must have a valid driver’s license
- Must reside within Anson, Montgomery, Moore, or Richmond County, North Carolina
- Must be able to travel within the catchment as required
- Preferred: Two (2) years paid experience working directly with individuals with BH/MH or IDD conditions
- Preferred: Paid experience related to the counties in which they are applying
- Preferred: Certified Peer Support Specialist
- Must pass a drug test as a condition of employment
