Job Description
Registered Dietitian in a mission-driven healthcare organization delivering personalized nutrition services. Conducting nutrition assessments, developing meal plans, and managing telehealth programs.
Responsibilities:
- Deliver Medical Nutrition Therapy (MNT) to patients with chronic conditions via video, phone, email, and portal.
- Conduct comprehensive nutrition assessments, develop individualized care plans, and provide counseling on dietary modifications.
- Use Medically Tailored Meals as educational tools to support patient learning and behavior change.
- Leverage Remote Patient Monitoring data and portal feedback to adjust treatment plans and optimize patient outcomes.
- Provide evidence-based lifestyle counseling, including behavior change strategies, motivational interviewing, and goal setting.
- Manage an independent caseload with 33-35 hours/week of scheduled telehealth appointments.
- Monitor patient progress through regular follow-ups, portal engagement, and RPM data.
- Maintain accurate, timely documentation in the EMR system following OnPoint Nutrition protocols and regulatory requirements.
- Ensure compliance with HIPAA, state licensure requirements, and professional practice standards.
- Collaborate with peers through case consultations and team learning.
Requirements:
- Minimum of bachelor’s degree in nutrition, dietetics, or a related field required.
- Registered Dietitian (RD) or Registered Dietitian Nutritionist (RDN) credential from CDR required.
- Active state licensure in AL, FL, NC, NJ, NY, PA, or VA, or the ability to obtain multi-state licensure, is required.
- 2+ years of clinical nutrition experience required.
- Experience with EMR systems required.
- Strong interpersonal and communication skills; comfort with telehealth technology required.
- Experience in chronic disease management and value-based care across underserved populations, including individuals with disabilities, uncontrolled diabetes, post-acute care needs, uncontrolled hypertension, and other complex conditions, preferred.
- Certification in obesity and weight management (CSOWM) or diabetes care and education specialist (CDCES) preferred.
