Job Description

Care Coordinator managing RPM/CCM care for healthcare practices across the US. Leading case management teams and ensuring integrated health service delivery.

Responsibilities:

  • Onboard new Practices and operationalize a Care plan for their patients
  • Manage a team of case managers – Delegate & monitor RPM and CCM-related targets assigned to each case manager
  • Coach and mentor case managers to improve productivity and care being provided to patients
  • Act as the key point of contact between the Neolytix team and the Practice Provider
  • Identify patients for specific case management and/or chronic condition management activities using established screening criteria
  • Apply nursing processes to actively manage members and achieve integrated health services through coordinated service efforts

Requirements:

  • At least 5 years of clinical experience with direct patient care required
  • Experience in case management/coordination of programs and services preferred
  • Bachelor of Science in Nursing (BSN) required
  • Excellent verbal and written communication skills
  • Strong administrative/technical skills; Comfort working on a PC using Microsoft Office (Outlook, Word, Excel, PowerPoint), IM/video conferencing (Teams & Zoom), and telephones efficiently
  • Trustworthy and accountable behavior, capable of viewing and maintaining confidential personal information daily.
