Job Description
Registered Nurse case manager coordinating admissions and ongoing infusion therapy care for CarepathRx. Collaborating with patients, providers, pharmacists, and home health agencies.
Responsibilities:
- Coordinate patient referrals and admissions
- Confirm patients meet admission and clinical requirements
- Communicate with patients, providers, pharmacists, home health agencies, and internal teams
- Provide ongoing case management, including help with infusion pumps, supplies, and care transitions
- Educate patients and caregivers about therapy needs and available resources
- Deliver responsive customer service and help improve team processes
Requirements:
- Graduate of an accredited nursing program
- Active Registered Nurse (RN) license in PA or Compact
- Strong communication, organization, and time management skills
- Proficiency with Microsoft Office
- Ability to manage multiple priorities in a fast-paced environment
- If working from home, cable broadband or fiber optic internet service with at least 10 Mbps download and 5 Mbps upload
- Bachelor of Science in Nursing (BSN) preferred
- Experience in home infusion, specialty pharmacy, case management, or home health preferred
