Job Description
RN Case Manager coordinating medical needs and wellness for CVS Health members in Louisiana. Conducting telephonic case management with required regional field travel.
Responsibilities:
- Coordinate all case management activities with members to evaluate medical needs
- Facilitate members' overall wellness
- Develop strategies to address issues, outcomes, and opportunities to enhance members' wellness through integration
- Instruct on programs and procedures in compliance with network management and clinical coverage policies
- Travel within 125 miles round trip for 1 day in the field
- Work a variable schedule, including 2 days weekly until 9pm local time
- Achieve performance metrics and maintain compliant documentation
Requirements:
- Must reside in Louisiana in one of the following parishes: Caddo, Bossier, Webster, Claiborne, Bienville, Red River, DeSoto, Sabine, or Natchitoches
- Current/Active Clinical Licensure (RN, LCSW, LPC, LMFT)
- Case Management experience
- Strong customer service orientation and problem solving
- Excellent Communication/Telephonic skills
- Excellent motivational interviewing skills/ability to build rapport and trust telephonically
- Excellence in documentation and respecting compliance/regulatory standards
- Highly organized and self-driven without direct supervision
- Ability to manage time effectively
- Ability to achieve performance metrics
- Technology proficiency
- Ability to travel within 125 miles round trip for 1 day in the field
- Variable work schedule with ability to work 2 days weekly until 9pm local time
- Bachelors and/or Master's Degree
- CCM Certification
- Previous experience in Managed Care Organization
