Job Description
Field Care Manager Nurse coordinating behavioral health care for Humana members in South Carolina. Assessing needs, managing care plans, and conducting field visits up to 50% of the time.
Responsibilities:
- Assess and evaluate members' needs and requirements to achieve or maintain optimal wellness
- Guide members and families toward appropriate care and wellbeing resources
- Facilitate interaction with care and community resources
- Manage members' physical, environmental, and psychosocial health issues using varied strategies and techniques
- Identify and resolve barriers to effective care
- Monitor patient care through assessments and evaluations
- Ensure patients progress toward desired outcomes
- Create member care plans as needed
- Apply department, segment, and organizational strategy and operating objectives
- Travel up to 50% to support collaboration, conduct face-to-face meetings, and engage with staff, providers, members, and families
- Report to the Manager, Care Management of Behavioral Health
Requirements:
- Bachelor's in nursing (BSN)
- Active license in the state of South Carolina without disciplinary action
- Must reside in the State of South Carolina
- 2 or more years of experience in case/care management
- 2 or more years working with the behavioral health population
- Knowledge of community health and social service agencies and additional community resources
- Ability to use electronic information applications/software programs, including electronic medical records
- Reside close to the Midlands OR Upstate area where your region will be
- Dedicated workspace without ongoing interruptions to protect member PHI / HIPAA information
- Self-provided internet service with at least 25 Mbps download and 10 Mbps upload speed
- Preferred: 3 or more years of in-home assessment and care coordination experience
- Preferred: Experience with health promotion, coaching and wellness
- Preferred: Previous managed care experience
- Preferred: Bilingual — English, Spanish
- Preferred: Certification in Case Management
- Preferred: Motivational Interviewing Certification or knowledge
