Job Description
Care Transitions Care Manager coordinating medical, behavioral, and social services for North Carolina residents. Supporting continuity of care across providers and community resources at Trillium Health Resources.
Responsibilities:
- Manage ongoing care for assigned individuals to support continuity and stability across services and settings
- Develop and maintain comprehensive care plans addressing medical, behavioral, and social needs
- Coordinate services across providers and community resources
- Monitor progress toward care goals and adjust care plans based on changing needs
- Engage individuals and families to support sustained participation in services and care plans
- Provide comprehensive care coordination for individuals transitioning between care settings and maintaining stability over time
Requirements:
- Must meet QP status through one of the stated qualification pathways: fully licensed by the NC governing board regulating Human Services professions; or fully licensed RN licensed to practice in North Carolina with four years of full-time MH/DD/SA experience; or a master's degree in a human service field with one year of full-time MH/DD/SA experience; or a bachelor's degree in a human service field with two years of full-time MH/DD/SA experience; or a bachelor's degree in another field with four years of full-time MH/DD/SA experience
- Substance abuse professional alternative pathways require the stated supervised alcoholism and drug abuse counseling experience
- If serving members with LTSS needs, minimum two years of prior LTSS and/or HCBS coordination, care delivery monitoring, and care management experience; may be concurrent with QP experience
- Must have a valid driver's license
- Must reside in North Carolina and within 75 miles of Trillium's Southern Region
- Eligible counties: Bladen, Brunswick, Columbus, Hoke, Lee, Moore, New Hanover, Robeson, and Scotland
- Must be able to travel within catchment as required
- All candidates must pass a drug test as a condition of employment
- Preferred: Degree in Human Services
